Superintendent of Financial Services REGARDING the Insurance Act, R.S.O. 1990, c. 1.8, as amended (the "Act"), in particular , sections 393(9) - 393(11)

AND REGARDING Daniel Mullins , life insurance agent

ORDER Pursuant to Minutes of Sett lement and attached Schedule A , all of which are filed, I he reby ord er the following sus pens ion and conditions on Mr Mullins' licence as an insurance agent:

1. The life insurance agent licence of Daniel Christopher Mullins, (licence nu mber 04083556) shall be suspended from May 26, 2010 until December 31,2010.

2. After December 31 ,2010, Mr. Mullins' license shall only be reinstated if: (a) Mr. Mullins provides proof satisfactory to the Superintendent that he has obtained errors and omissions coverage insurance in accordance with Ontario Regulation 347/04.

(b) Mr. Mull ins has completed a course dealing with profess ional ethics, responsibilities, and compliance and provides the Superintendent with proof of satisfactory completion of that course. Mr . Mullins shall obtain approval of that course beforehand from the Superintendent and shall pay any costs associated with that course. This requirement shall be in addition to the continuing education requirements imposed by Ontario Regulation 347/04.

3. For a period of eighteen months, commencing on the date that Mr. Mullins' licence is reinstated , Mr. Mullins shall mail a letter to the Superintendent each

month , bearing a postmark not later than the fifteenth day of each month , listing all insurance applications he submitted to insurance companies during the preceding month. The letter shall not include the actual applications.
