Council determined that this Life Insurance Agent be fined in the amount of
$1,000.00 and assessed costs in the amount of $1,000.00 for failure to complete
a disclosure statement, failure to disclose intended replacement on an
application, and facilitating the cancellation of a policy of life insurance
prior to ensuring proposed coverage was in effect. These actions were in violation
of The Insurance Act of Manitoba; Section 113(1)(e) and 375(1)(a) and
the Life Insurance Agent Code of Conduct, Sections 3 and 6.
