REGARDING the Insurance Act, R.S.O. 1990, c. I.8, as amended (the “Act”), in particular, sections 393(9) – 393(11)

 

AND REGARDING Carrie Lynn Sullivan

                                   

 

ORDER

 

 

 

 

Pursuant to Minutes of Settlement, I hereby order the Life Insurance and Accident and Sickness Agent Licence of Carrie Lynn Sullivan (licence number 02071332) shall be suspended from November 1, 2014, until July 31, 2015.
