REGARDING the life insurance and accident & sickness licence of Joseph RAWN Licence# 11122848 (Hereinafter referred to as “the agent”)
AND REGARDING the Insurance Act, R.S.O. 1990, c.I.8 as amended (The “Act”)
ORDER
Pursuant to a Minutes of Settlement and Agreed Statement of Facts, listed as Schedule “A”, all of which are filed, I hereby order that the Life Insurance and Accident & Sickness Licence of Joseph RAWN number 11122848 be revoked commencing May 10, 2013.
