REGARDING the life insurance and accident & sickness licence of Michael AXON Licence# 06090811 (Hereinafter referred to as “the agent”)

AND the Insurance Act, R.S.O. 1990, c.I.8 as amended (The “Act”)

 

ORDER

 

Pursuant to a Minutes of Settlement and Agreed Statement of Facts, listed as Schedule “A”, all of which are filed, I hereby order that the Life Insurance and Accident & Sickness Licence of Michael Axon number 06090811 be suspended for a period of six  (6) months commencing April 1, 2013.
