SUPERINTENDENT OF FINANCIAL SERVICES REGARDING the life agent license of Michel J. LACHAUME AND the Insurance Act R.S.O. 1990 Chapter I.8, as amended, particularly section 393. ORDER Pursuant to Minutes of Settlement and the Agreed Statement of Facts listed as Schedule “A” all of which are filed, I hereby order that the life insurance license of Michel J. Lachaume

1. Be suspended for a minimum period of three (3) months, effective this date. 2. In no circumstances will the suspension Mr. Lachaume’s licence be lifted until he provides evidence, acceptable to the Superintendent, of completion of 30 hours of continuing education applicable to the licence term expiring on December 2, 2009.

3. By February 09, 2012, Mr. Lachaume provide evidence acceptable to the Superintendent of the completion of 30 hours of continuing education applicable to the licence term expiring on February 09, 2012, the last day he may be licensed without educational re-qualification.
