JOHNSON AND the insurance act R.S.0. 1990 Chapter 1. 8, as amended, particularly section 393. ORDER Pursuant to Minutes of Settlement and Agreed Statement of Facts listed as Schedule "A", all of which are filed, I orderthat the insurance license of Pauline M. JOHNSON be suspended for a period of forty-five (45) days effective February 1,2009.

I) Pauline Johnson was issued a life insurance license by the Superintenden t of Financial Services on July 15 th 1994. This license was numbered 94023708 and has been renewed every 2 years thereafter as required.

2) Johnson has no prior record of complaint nor misconduct. 3} On or about January 1t h 2008, info rmation was provided to the Superintendent of Financial Services that Johnson had been terminated by the Co-operators General insurance Company for allegations involving forgery of cl ient signatures.

4) Johnso n forged the signature of Velma Blanch ard on investment documents. 5) Johnson forged the signature of Steve Lyle on investment doc uments. 6) Johnson forged the signature of Anne MacLeod on investmen t documents. 7) It has been determined that no finan cial harm was suffered by any consumer or cl ient named nor comp any invol ved .

8) It has been determined that in each case, Johnson signed the consumer or cli ent ' s name for the purpose of expediency.

9) Johnson attended the offices of the Financial Services Commission of Ontario to answer to the allegations and adm itted the circumstances and expressed her regret and remorse. ~
