REGARDING the Insurance Act, R.S.O. 1990, c.I.8, as amended (the “Act”), in particular sections 392.5 and 407.1;
AND REGARDING Anthony Paul Dorzek
ORDER TO REVOKE LICENCES
On November 23, 2018, the Superintendent of Financial Services (“Superintendent”) issued a Notice of Proposal to Revoke Licence (the “NOP”) to Anthony Paul Dorzek (“Mr. Dorzek”) regarding a general insurance agent licence and a life and accident and sickness insurance agent licence, both of which were issued to Mr. Dorzek under the Act (licence #08100995).
The NOP was served on Mr. Dorzek on November 28, 2018. The Financial Services Tribunal has confirmed that Mr. Dorzek did not request a hearing within 15 days as required by subsection 407.1(3) of the Act.
Subsection 407.1(7) of the Act states that if the licensee does not request a hearing, the Superintendent may carry out the proposal.
ORDER
The general insurance agent licence and the life and accident and sickness insurance agent licence of Anthony Paul Dorzek (licence #08100995) are both hereby revoked.
