AND REGARDING Under Pressure Hyberbaric Treatment Centre Inc. [SP16172] (the “Provider”)
ORDER VARYING AN ORDER IMPOSING SUMMARY ADMINISTRATIVE PENALTIES
This Order varies the Order Imposing Summary Administrative Penalties dated February 8, 2018, issued by delegated authority from the Superintendent of Financial Services against Under Pressure Hyperbaric Treatment Centre Inc. (attached as Appendix “A”).

Pursuant to s. 16(2) of the Act, and on consent of Under Pressure Hyperbaric Treatment Centre Inc., the Superintendent is varying the Order dated February 8, 2018 to read as follows:
ORDER IMPOSING SUMMARY ADMINISTRATIVE PENALTY
The Provider holds a health care services provider licence. In a letter, dated November 21, 2017, sent to the Provider, a delegate of the Superintendent of Financial Services (“Superintendent”) advised of the intention to impose a summary administrative penalty of $1,000 against the Provider for failing to file the 2016 Annual Information Return by March 31, 2017, as required by subsection 21(1) of Ontario Regulation 90/14 made under the Act.
Pursuant to subsection 441.4(2) of the Act, the Superintendent gave the Provider the opportunity to make written submissions, which were to be received by the Superintendent’s delegate within the time specified in the letter. The Provider’s submissions were reviewed and the Superintendent has decided to proceed with the imposition of the summary administrative penalty.
Therefore, pursuant to subsection 441.4(1) of the Act and Item 53 of Schedule 3 of Ontario Regulation 408/12, the Superintendent orders a summary administrative penalty against the Provider.
Pursuant to s. 16(2) of the Act, and on consent of Under Pressure Hyperbaric Treatment Centre Inc., the Superintendent is varying the Order dated February 8, 2018 to read as follows:
ORDER
A summary administrative penalty totalling $1,000 is imposed on Under Pressure Hyperbaric Treatment Centre Inc. [SP16172].
TAKE NOTICE THAT the Provider will be receiving shortly an invoice from Ontario Shared Services, a part of the Ministry of Government and Consumer Services, with information as to where and how to make the payment. The Provider must pay the administrative penalty no later than 30 days after the date of the invoice.
If the Provider fails to pay the administrative penalty in accordance with the terms of this Order, the Superintendent may file the Order with the Superior Court of Justice and the Order may be enforced as if it were an order of the court. An administrative penalty that is not paid in accordance with the terms of an order imposing the penalty is a debt due to the Crown and is enforceable as such.
