AND REGARDING Farima Farhat
REVOCATION ORDER
On August 25, 2015, the Superintendent of Financial Services (“Superintendent”) issued a Notice of Proposal to Revoke Licence (“Notice”) regarding the life insurance agent and accident and sickness insurance licence of Farima Farhat (“Ms. Farhat”).
Ms. Farhat received the Notice on September 1, 2015, and had 15 days after the date the Notice was given to request a hearing before the Financial Services Tribunal (“Tribunal”) in accordance with subsection 407.1(3) of the Insurance Act (the “Act”).
The Registrar of the Tribunal has confirmed that no request for a hearing has been received from Ms. Farhat or anyone on her behalf in respect of the proposal to revoke.
Subsection 407.1(7) of the Act provides that the Superintendent may carry out the proposal to revoke a licence when no hearing has been requested.
ORDER
The life insurance and accident and sickness insurance agent licence of Farima Farhat is revoked.
