IN THE MATTER OF the Insurance Act, RSO 1990, c. I.8, as amended (the “Act”), in particular sections 288.5 and 288.7;

AND IN THE MATTER OF Parkside Village Therapy Inc. (“Parkside Village”).

ORDER TO REFUSE TO ISSUE LICENCE

On January 17, 2020, Parkside Village submitted an application for a service provider’s licence under the Act.

On June 18, 2020, by delegated authority from the Chief Executive Officer of the Financial Services Regulatory Authority of Ontario (the “Chief Executive Officer”), the Director, Licensing Approvals, issued a Notice of Proposal to refuse to issue a service provider’s licence to Parkside Village.

A Request for Hearing (Form 1), dated June 18, 2020 was delivered to the Financial Services Tribunal (the “Tribunal”) in accordance with section 288.7(3) of the Act respecting the Notice of Proposal.

On March 26, 2021, Parkside Village withdrew the Request for Hearing, and on March 30, 2021, the Tribunal closed its file in respect of this matter. Therefore, pursuant to section 288.7(7) of the Act, and by delegated authority from the Chief Executive Officer, the Head, Licensing and Risk Assessment, makes the following order.

ORDER

The issuance of the service provider’s licence to Parkside Village Therapy Inc. is hereby refused, for the reasons set out in the Notice of Proposal.
