IN THE MATTER OF the Insurance Act, R.S.O. 1990, c.I.8, as amended (the “Act”), in particular sections 392.5 and 407.1;
AND IN THE MATTER OF Salome Caoile.
ORDER TO REVOKE LICENCE
On March 11, 2019, the Superintendent of Financial Services (“Superintendent) issued a Notice of Proposal to Revoke Licence to Salome Caoile (“Ms. Caoile”) regarding insurance agent licence #17162163 issued under the Act.
On March 15, 2019, the Notice was received by Ms. Caoile. Ms. Caoile had 15 days from the date the order was given to file a request for hearing with the Financial Services Tribunal (the “Tribunal”).
On June 8, 2019, the Financial Services Regulatory Authority of Ontario assumed the regulatory duties of the Financial Services Commission of Ontario, and the Chief Executive Officer of the Financial Services Regulatory Authority of Ontario (the “Chief Executive Officer”) assumed the regulatory duties of the Superintendent, under the Act.
The Registrar of the Tribunal confirmed on April 3, 2019 that no request for hearing had been made by Ms. Caoile or anyone acting on her behalf.
ORDER
The insurance agent licence #17162163 issued to Salome Caoile is hereby revoked.
